In accordance with the Courtyards of Vinings Homeowners Association Covenants, Easements, Conditions and Restrictions (Declarations) and the Association's Rules and Regulations, I request your consent to make the following changes, alterations, renovations and/or additions to my home and/or lot: (please add a brief description of what, why, when, where and how.)

Include site plans, drawings, blueprints, etc.  Furnish enough detail so that the CV Board can identify exactly what your plan entails and its intended location. Two (2) copies are required of all attachments.  One (1) copy will be returned and will indicate APPROVAL or DISAPPROVAL.

I understand that the approval by the Courtyards of Vinings Board of Directors is in no way to be construed as a Guarantee that the approved projects meet Local and State codes or safety regulations.
